Please use pen, no pencils. Please answer all questions. Please come in your soccer gear ready to play. Bring your soccer ball and a water bottle. You must be accompanied by a Parent or Guardian, and they must sign an AYSO insurance waiver.
The next AYSO Matrix Board meeting will be at Hourglass Park in Mira Mesa on August 9, 2010. The meeting will start at 7:00 pm.

We are looking for a few more U15B players for the Mira Mesa Matrix team, we have been seeded in AA-BB North. Any position would be considered. We practice Monday and Thursday evenings from 6pm to 7:30pm in Mira Mesa. They can contact me direct at: gjasinski@san.rr.com and 858-337-0377.
